Open enrollment is here for most employer health plan!

Family employer premiums hit $27,000 in 2025 (KFF). Workers paid about $6,850 of that out of pocket. And Mercer is reporting 2026 will be the biggest cost increase in 15 years. Last year’s plan may not be the best plan this year.

Families paying $1,500+ a month to add a spouse or kids, healthy enough to qualify for private underwriting, or looking for PPO flexibility, comparing a private plan is often worth the half-hour conversation.

đŸš© No maximum out-of-pocket in writing

Marketplace subsidies aren’t really a discount, they’re an advance, based on what you estimated you’d earn. If you earn more than you guessed, the IRS expects part of it back.

For self-employed people whose income fluctuates, this is one of the largest tax-time surprises in the country.
And starting with 2026 plans, the rules just changed.

Here’s the part most people don’t know: you don’t have to wait until April to find out the damage. Private health insurance doesn’t follow ACA open enrollment. You can apply mid-year. If approved, you cancel your Marketplace plan and the subsidy clock stops.

If your 2026 income is already tracking higher than what you projected at open enrollment, the longer you stay on Marketplace, the bigger the bill becomes at tax time.

Everybody shops health insurance by monthly premium
 until they actually need to use the plan.

The cheapest premium is usually the most expensive plan when you start dealing with deductibles, coinsurance, limited networks, and high out of pocket costs.

A low monthly payment does not always mean better coverage.

Before choosing a plan, compare:

Deductible
Max out of pocket
Network
Prescription benefits

Most self-employed people don’t know they have two completely different paths for health insurance.
There’s the Marketplace (Healthcare.gov) — and there’s private (off-exchange) health insurance, sold directly through carriers and agents. They follow different rules, and the right one depends entirely on your situation.

Marketplace can be excellent if you qualify for subsidies or if you’re managing a pre-existing condition. Private PPO plans can be a smarter fit if you’re healthy, your income fluctuates, or you want a wider network and don’t want to tie your coverage to a tax-return forecast.

There’s no single “best” health insurance for self-employed people. There’s only the best plan for your income, your health, and the way your business actually works.

Save this for the next time you’re thinking about your coverage. Send it to the freelancer in your life who needs to read it.

“My COBRA quote is 1,200 a month. I cannot afford this. What do I do?”

And almost every time, the person on the other end of that call has no idea there is a third option.
Most people think you either keep your old plan through COBRA or you go to Healthcare.gov for a subsidy. Those are the two paths everyone talks about.
But there is a third path. Private PPO plans, sold off marketplace, with flat pricing not tied to your income. For healthy people, the math often looks like this:

COBRA 800 to 1,000 a month
Marketplace PPO 550 to 700 a month
Private PPO 350 to 450 a month

Same person. Same coverage need. Very different bill.
The catch is honest. Private plans are medically underwritten, so they are not a fit if you have chronic conditions, specialty meds, or active treatment. In that case COBRA or Marketplace is usually the smarter call.

If you just lost your job, you have 60 days to decide. That is enough time to actually compare instead of panic enrolling.
