Hailey’s words: Here’s a little scenario for you guys to help guide you if you ever are uncertain about a doctors orders. I was definitely afraid to speak to doctors when I first became a nurse, but it got easier the more I got to know the doctor.

This method won’t always work because sometimes you come across some doctors who in general are just terrible to work with. Thankfully I have not come across any doc like that.🙏

Remember, always approach the situation with the intent to learn! If you’re open and honest about you want and need to learn, most doctors will be happy to give you a quick little rundown of their thought process.🧠

🩸Gi bleeds can sometimes be very bad, or very minor! Naturally, we always hope for the later. Read on for some more information on GI bleeds⁣

𝙏𝙮𝙥𝙚𝙨 𝙤𝙛 𝙂𝙄 𝘽𝙡𝙚𝙚𝙙𝙨:⁣
𝙐𝙥𝙥𝙚𝙧 𝙂𝙄 𝘽𝙡𝙚𝙚𝙙: Bleeding that occurs in the upper part of the digestive system, usually the esophagus, stomach, or duodenum. The blood is darker with upper gi bleeds as it is partially digested.⁣
𝙇𝙤𝙬𝙚𝙧 𝙂𝙄 𝘽𝙡𝙚𝙚𝙙: Bleeding that occurs in the lower part of the digestive system, typically the colon, re**um, or anus.⁣

GI bleeding can be caused by various factors, including:⁣
𝙋𝙚𝙥𝙩𝙞𝙘 𝙐𝙡𝙘𝙚𝙧𝙨: Sores that develop on the lining of the stomach or the upper part of the small intestine. This is the most common.⁣
𝙂𝙖𝙨𝙩𝙧𝙞𝙩𝙞𝙨: Inflammation of the stomach lining.⁣
𝘌𝘴𝘰𝘱𝘩𝘢𝘨𝘦𝘢𝘭 𝘝𝘢𝘳𝘪𝘤𝘦𝘴: Dilated blood vessels in the esophagus, often seen in advanced liver disease.⁣
𝘿𝙞𝙫𝙚𝙧𝙩𝙞𝙘𝙪𝙡𝙤𝙨𝙞𝙨: Small pouches in the colon can sometimes bleed.⁣
𝘾𝙤𝙡𝙤𝙧𝙚𝙘𝙩𝙖𝙡 𝙋𝙤𝙡𝙮𝙥𝙨 𝙤𝙧 𝘾𝙖𝙣𝙘𝙚𝙧: Abnormal growths in the colon or re**um.⁣
𝙈𝙚𝙙𝙞𝙘𝙖𝙩𝙞𝙤𝙣𝙨: Some drugs, NSAIDs may cause GI bleeding.⁣

In my opinion, bleeds stemming from esophageal varices are hands down the worst and I have seen patients pass from them bursting. ⁣

If the GI bleed cannot be treated and handled with medication, they will typically do an endoscopy to help stop the bleeding as well as diagnose where exactly it is coming from⁣

⚠️When dealing with hyponatremia & 3% Sodium Chloride, treatment will vary based on acute vs. chronic and the cause of the hyponatremia. 3% NaCl is just one treatment available. ⁣

▪️Typically, the more acute the hyponatremia, the greater the risk of complications (such as cerebral edema and seizures) and the greater the need for aggressive therapy. The more chronic the hyponatremia and the lower the serum sodium concentration, the greater the risk of complications from overaggressive therapy and the greater the need for monitoring to avoid overcorrection.⁣

▪️The target for correcting hyponatremia typically involves raising the serum sodium concentration by 4 to 6 mEq/L within the first 24 hours of therapy. In symptomatic cases or severe situations, the goal may be achieved quicker but the recommended maximum rate correction is no more than 8 mEq in 24 hours
