We opened up an exterior vent last week and found a bird had beaten us to it.

A nest had packed the termination almost completely shut, so hot moist air had nowhere to go but back into the wall cavity.

Once we cleared it and replaced the termination with a proper vent hood, airflow was back to normal and the risk was gone.
