Before accomplishing the form, please prepare clear scanned copies or photos of the following:

📸 Pupil’s Latest ID Photo (2" × 2", white background)
📄 PSA Birth Certificate
📚 Official Report Card or DepEd Form 138 (at least up to the 2nd Quarter) – for Grade School applicants
🤝 Certificate of Good Moral Character from the previous school – for Grade School applicants

📘 Passport Bio-page and Latest Admission Stamp with Valid Authorized Stay (for both parent/s and applicant)

AGE REQUIREMENT FOR PRESCHOOL

Playschool: Child must be 3 years old on or before August 31, 2026
Nursery: Child must be 4 years old on or before August 31, 2026
Kindergarten: Child must be 5 years old on or before October 31, 2026

Note to Kindergarten Applicants: Children born between November 1 and December 31 are required to comply with the Early Childhood Care and Development (ECCD) Checklist. This checklist is available at the Wellness and Testing Center (WTC).
