Many of you have been making a kind donation towards this latest “Carby’s Collectables”Glass.

Below is a detailed description of what exactly 100% of your donation is going to assist.

“2020 Vision for Patient Transition”
Shriners Hospitals for Children
Springfield has identified a need to assist older patients with care after they age out of treatment at the Hospital.

This project will focus on multi-complex patients, age 14 and older, and help them to develop a plan for after they move on from Shriners Hospital’s care.

As patients transition from Shriners Hospital care, they should be as independent as possible in managing their own care. Patients should know their doctors, dentists, allergies, and medication list. Patients will be guided to resources for items such as equipment, orthotics and prosthetics and support services.

As patients “graduate” from care, special gift bags will be created for each patient and a special party will be held.

The ultimate goal of this project is to help patients continue to access their needed medical care outside of Shriners Hospital. When patients leave the hospital, they will be confident and be ready to access services in the community.

100% from your donation goes directly to The Interfaith Emergency Shelter.

An overflow shelter for Grove Street Inn. Guests are provided with warm beds and hot meals from November through April. The 20-bed site is located in the basement of a professional office building at 43 Center Street in Northampton (plus a 6-bed annex in Easthampton). The shelter is a cooperative effort among the City of Northampton, the 400+ volunteers of the Friends of the Hampshire county Homeless Individuals, and ServiceNet, which provides professional staffing.

100% of your donations towards this glass will go directly to support The Melha Shriners “ 1st Lady’s Project” of Tricia Plourde.
