Some bacteria don't follow the rules. Atypical bacteria like Chlamydia, and Legionella don't have a normal cell wall, which means they don't take up Gram stain, don't grow on standard culture plates, and don't respond to the treatments most commonly used on bacterial infections.

Molecular testing reads microbial DNA directly, so these bacteria don't get overlooked.

We read all three of Zimmer Biomet's instructions for use for Synovasure.

All three say the same thing:

"Not intended to identify the etiology or severity of a PJI."

Their regulatory text. Not our characterisation.

• Alpha Defensin ELISA, Alpha Defensin Lateral Flow, SynTuition Score. One identical sentence across all three.

• In the demo case CD Laboratories publishes as its own sales collateral, the score returns inside 24 hours reading high probability of infection.

• The organism, Staphylococcus epidermidis, oxacillin resistant, arrives on the culture line, which that same report twice states is excluded from the score.

• Cultures are held 7 days. 14 for shoulder specimens.

• Their own 2018 ICM brochure already says it: "consider further molecular diagnostics such as next-generation sequencing."
