- A hardwood bar found by a follower – Eye bolts through the bar and instead of shackles and proper wire, they have tied steel wire rope and used multiple wire rope grips to lock it. This bends the wire rope far past it's safe bend radius and greatly weakens the wire. No counterweights were present anywhere on this line. Get your rigging inspected, or shop for new rigging at performance.wengercorp.com

- The handline on a counterweight system should never cross or touch the wire rope lift lines. Contact can create friction, which may cause the wire rope to wear through the handline and lead to failure. Get your rigging inspected, or shop for new rigging at performance.wengercorp.com

"We provide laundry baskets during shows that require characters to perform quick costume changes. Each actor has an assigned laundry basket labeled with their name and character. Crew members are ready when the actor comes off stage. They assist the actor in changing their costume their current costume comes off quickly and into the basket...the new costume is put on so the actor can return to the stage. If the costume is needed again in the show, the crew can re-hang or prepare the original costume in the basket. If not the baskets are stored right under the fly rail, out of the way from people moving around backstage. At the end of the show all costumes are rehung and baskets are empty." -Ken L. - Using an unmoused shackle on an “S” hook to hang a curtain track is unsafe. The shackle pin can loosen without mousing, and “S” hooks aren’t rated for overhead loads.
